Read the full vitals table over time and explain the clinical story: fever, blood pressure, oxygen saturation, heart rate, and documentation consistency.
I&O Tracking
Calculate intake, output, and net fluid balance, then decide how much trust to place in totals when the chart contains documentation gaps.
Medication Safety
Follow a Vancomycin trough across shift change, check acetaminophen totals, review active medication orders, and find IV fluid documentation without a matching order.
